Who is eligible?

All eligible riders must reside in Wellesley, Wilmot & Woolwich Townships. If you answer “YES” to ONE of the statements below, Kiwanis Transit may be able to help you (or someone you know) with transportation needs.

  • 65 years of age or older (proof of age required)

  • CNIB registrant (proof of registration required)

  • Physically disabled

  • Developmentally disabled

  • Have a temporary disability that affects mobility

APPLICATION FORMS

Print off all required forms, fill them out and fax them to Kiwanis Transit at 519-669-3124, or email us at kiwanistransit@k-transit.com.

To complete these forms digitally, please download the PDF document to your computer before filling in your information. Once complete, press ‘Submit’ on the form and follow the email instructions provided.

We will let you know as soon as possible if you qualify for our services.

mobility assessment form

Mobility Assessment Forms are required by all applicants under the age of 65 and/or as required by the transit. 

Please print this form and have it completed by a medical professional and submit this assessment form with your application using our fax number or scan it and submit both forms digitally.

VETERAN pass application

Canadian Veterans (over the age of 65), are eligible to receive a lifetime GRT pass for unlimited use of the service.
